Food Service Refund Form
Please note:
  • Unused funds carry over to the new school year, regardless of a student's ZCS building assignment. 
  • For those students graduating OR leaving ZCS, account balances greater than $5.00 will be refunded upon request. 
    • Please use form below. 
  • Refund Requests submitted over the summer will be processed in August and mailed in August/September. Thank you. 

Lunch Account Information

  • Checks can be made out to "ZCS Cafes" 

    • We are also happy to split one check among several students!

      • Please note the  PIN # of each student on your check!

You can apply online OR via a paper application, both options provided below. 
 
Paper applications are sent to the Food Service Office at Zionsville WEST Middle School, at 5565 South 700 East, Whitestown, IN 46075. 
 
Notification of results for anyone applying may not be received until July 27, 2018. Thank you!
