*Note – Medical billing separates provider and facility charges. It is the covered member’s responsibility to request an in-network provider and facility prior to having services performed in order to receive the in-network benefit. You can contact UGS directly at 800-291-5837 or visit https://www.unifiedgrp.com/ to verify if a provider is in-network or out-of-network. For the network, any member can call 317-338-CARE anytime to connect with a patient care line for help. This number gets a person to our centralized call center.

ZCS Wellness Center Information


Anyone covered under the ZCS health plan has free access to the wellness center including all of its services such as office visits, lab work and prescription medication.

Lab Work

Under the Silver and Eagle Plan, lab services will be covered 100% at the ZCS Wellness Center. For the Silver Plan, lab services will be covered 100% when Ascension St. Vincent’s, MACL/Quest (http://www.maclonline.com/), Ameripath, and ZCS Wellness Center facilities are utilized. For the Eagle Plan, the deductible must be met first before the plan will pay 100%. It is the covered member’s responsibility to inquire where providers send lab work.
