Dear Pleasant View Elementary Parents,

Attached please find a form that we are requesting you fill out for each PVE student.  The form will allow our school nurses to enter student immunizations in the CHIRP computer data base program.  CHIRP stands for the Indiana State Department of Health's Children and Hoosiers Immunization Registry Program.
